LISTED BELOW ARE A GENERAL LIST OF PHYSICIAN REFERRAL 

Dr. John Buchanan, DO, Team Physician- Northeast Georgia Physicians Group

Dr. Buchanan ‘Dr. Buck’- is a board-certified sports medicine physician who specializes in nonsurgical orthopedics. Dr. Buchanan graduated from Mercer University where he also lettered for four years in NCAA Division I baseball. He went on to complete a secondary undergraduate degree at Florida International University. Dr. Buchanan earned his Doctor of Osteopathic Medicine degree from Nova Southeastern University followed by nine years in the United States Army. While in the Army, he completed an internship at Walter Reid Medical Center and also served as a flight surgeon. He completed his post-graduate education at the University of Alabama and the University of Notre Dame. Dr. Buchanan serves as the team physician for Mill Creek High School and has since its opening in 2004.

Dr. Kellie K. Middleton, M.D.,

Dr. Middleton is an orthopedic surgeon who places great emphasis on providing advanced, comprehensive orthopedic and sports medicine care. She specializes in complex shoulder, elbow and knee having trained at a top orthopedic fellowship program in the nation. She is available in Lawrenceville for orthopedic surgery consultations on any injuries requiring possible surgical intervention or care.

Dr. Brian Morgan, M.D.,

Dr Morgan is board certified by the American Board of Orthopedic Surgery, with an additional subspecialty certification in sports medicine. Fellowship trained in sports medicine, Dr. Morgan’s practice includes general orthopedics, sports medicine, fracture care, arthroscopic surgery and work injuries. Dr. Morgan has special interest in knee, shoulder and hip arthroscopy. He is available in Gwinnett and Johns Creek locations for orthopedic surgery consultations on any injuries requiring possible surgical intervention or care.

Dr. Golan, M.D.,

Elan Golan, MD, is an orthopedic surgeon with expertise in the diagnosis and management of athletic injuries. Practicing out of Gwinnett County at the Northside Orthopedic Institute’s Snellville, Georgia office, Dr. Golan offers cutting-edge techniques to treat hip painknee painshoulder pain, and other types of pain caused by sports injuries

DR. NOTES- MEDICAL CLEARANCE

If at any time an athlete needs to or has been seen by a physician (MD, DO, PA, PAC), the athlete is not cleared to participate in practice or competitions until he/she returns a medical clearance note to the athletic trainer releasing them for clearance in physical activity. Medical clearances will not be accepted by physical therapists (PT, DPT, MPT, etc.) or chiropractors (DC) unless otherwise stated on initial note.
